Good Faith Estimate

Under the No Surprises Act, individuals who are uninsured or who choose not to use insurance have the right to receive a Good Faith Estimate of expected charges for services. If you are self-pay, you may request a written estimate of the cost of services prior to beginning treatment. The estimate will include the expected cost of services based on the information available at the time and is not a contract or guarantee of final charges. If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.

For questions about your right to a Good Faith Estimate or the dispute process, you may visit www.cms.gov/nosurprises. Additional details regarding fees and estimates will be reviewed during the consultation process.
